What is Frozen Shoulder?

Frozen shoulder syndrome is a very painful and debilitating condition of the shoulder characterised by pain and severe stiffness of more than 50% in all directions, (i.e. you can’t move the arm freely in any direction, especially behind your back).


Frozen Shoulder can be very distressing as it can makes you feel mildly disabled. You may well be suffering from:


  • Severe night pain and waking
  • Inability to lie on your side at night
  • Can’t brush your hair
  • Can’t brush your teeth
  • Can’t reach to your back pocket
  • Can’t lift your arm
  • Increased sweating

Frozen Shoulder can last for months!

Frozen Shoulder is a clinical diagnosis and is only very rarely the result of an underlying disease. Fortunately (and curiously) once cured it (almost) never comes back on the same shoulder.


It often comes on for seemingly no reason at all. In rare cases. Frozen Shoulder may occur follow a trauma, a shoulder surgery and sometimes following breast reconstruction.


In 1998 I developed a self-help technique for treating Frozen Shoulder. Following successful clinical trials, The Niel-Asher® technique was adopted by osteopaths and physicians in the UK, Europe, and the United States.

The Niel-Asher Technique

The Niel-Asher Technique™ is a ‘natural’ method utilizing the body’s own healing mechanisms. No drugs, no surgery. It is HANDS-ON-ONLY. It uses a specific and unique sequence of manipulations and pressure points to the shoulder joints and soft-tissues. In essence, these can be thought of as ‘inputs’ into the nervous system.

“The Big Freeze”

The start of Frozen Shoulder can be deceptive, a minor twinge or nagging pain might suggest you have slept awkwardly. People assume it will wear off, as the pain increases, one becomes stiffer and the range of movement diminishes. Before long, the shoulder seizes up and you are 'frozen'. In this case, the condition can be so debilitating that sufferers cannot walk, stand, sit, lift anything or even wash, without shuddering spasms of pain.


Why a shoulder locks is not clear. Sometimes, it follows an injury or repetitive exercise. More women than men are affected. Mostly, one shoulder seizes up, but two can be affected. The shoulder is a complex structure that interconnects with other bones and muscle tendons - think of the movements involved in reaching for a wallet in a back trouser pocket. All joints are bathed in a capsule or sack of lubricating fluid and, at rest, the shoulder fluid settles in the armpit. At the start of the freezing process, it seems that inflamed cells move into the capsule. Gradually, these cause obstructions that stick together, making it difficult to move.

Traditional Treatment Approaches have Failed

For many, the idea of painful injections or surgery is unacceptable, especially as all the evidence to date points to the fact that 'Intra-articular cortizone injections may reduce pain or increase mobility briefly, but can cause serious unwanted effects. Neither manipulation under anesthesia (which risks significant injury) nor surgery has any clear place in management' (UK Drugs & Therapeutic Bulletin Nov. 2000) The Niel-Asher Technique™ is hands-on. It is a 'natural' method tapping in to the body’s own healing mechanisms. No drugs, No surgery. It utilizes a specific sequence of manipulations to the shoulder joint and its soft-tissues.

Hospital Study & Clinical Trials

In a hospital study the technique was found to be effective 'over and above' traditional physical therapy for increasing (active) range of motion, decreasing pain and improving strength and power.

Study Conclusions

The Niel-Asher Technique™ demonstrated a significant improvement in active range of movement, and strength and power over and above traditional physiotherapy treatment, in an outpatient hospital setting. Patients treated with the Niel-Asher Technique™ also showed the largest clinical improvement in shoulder pain and disability.

Home Healing Program

Frozen Shoulder Fact File

  • More common in women (60%)
  • At least five times more common in diabetics
  • Slightly more common in patients with Dupytren’s contracture and shares some of the same pathology
  • May have a genetic component i.e. it can run in the family
  • Seems to affect 40-70 year olds
  • About 15% of people develop frozen shoulder on both sides (commonly within 6-12 months of the first occurrence)
  • Can occur after shoulder surgery
  • Can occur after breast reconstruction surgery
  • It can, rarely, be a precursor for other pathologies
